CLINIC LOCATIONS
Our main office is located at Mercy Plaza, Suite 153, 1225 South Gear Avenue, West Burlington, IA 52655. This is in the medical office complex attached to Great River Medical Center. Our office hours are 9:00 a.m. – 5:00 p.m., Monday – Friday. We are closed for lunch from 12:00 – 1:00 p.m. All scheduling is done through our West Burlington office. Our billing and insurance staff are located at this office. Our office staff will be able to assist you with any questions regarding your diagnosis, treatment, medications, and test procedures. For the convenience of our patients and referring physicians, we have outreach clinics at:

1. Ft. Madison Community Hospital, at the Specialty Clinic which is adjacent to the FMCH Family Practice entrance.
   We have a clinic from 10:00 a.m. – 5:00 p.m. on Thursdays staffed by Dr. Alkurdy and Dr. Dhuna. View Map
2. Keokuk Area Hospital, we have a clinic at 1603 Morgan (Dr. Holsinger’s office) on Monday afternoons from 1:00 p.m. – 5:00 p.m. and Wednesdays from 10:00 a.m. - 5:00 p.m. View Map
3. Henry County Health Center at the Specialty Clinic in Henry County Medical Center on Mondays from 9:00 a.m. – 12:00 noon and Thursdays from 9:00 a.m.– 12:00 noon. View Map
4. McDonough District Hospital, we have a clinic at 903 East Grant Street, (Dr. Sherry Li’s Office) on Tuesdays from 10:00 a.m.– 5:00 p.m. and on Fridays from 10:00 a.m. - 3:30 p.m. Our Macomb number is 309-836-6772. View Map

Our offices will be closed on public holidays including New Years Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, and Christmas.

INSURANCE

On the day of your appointment, please bring your insurance card and any other pertinent insurance information so this can be submitted to the proper insurance company. As a courtesy to you we can file your medical claim to your insurance carrier.

If you are involved in a work related injury or an accident, please supply the office with complete billing information. Written authorization from your employer or adjustor is required prior to the date of service.

Co-payments are collected at the time of service.

Prompt payment is required for any outstanding balance. Payment is due 30 days after the day of service and the balance is billed at a standard 1.5% interest rate (see co-payments below). Burlington Neurology & Sleep Clinic, PLC, participates with Medicare, Iowa and Illinois Medicaid, Wellmark Blue Cross Blue Shield of Iowa, Alliance Select, John Deere Heritage, Sisco, EBS, Health Alliance, and Quincy health group.

For private patients we will collect a minimum of $100 at the time of service with the remaining balance due within 30 days. We also offer a 10% prompt payment discount for services paid in full at the time of service.

CO-PAYMENTS AND DEDUCTIBLES

Co-payments and deductibles are due at the time of service. We accept payment in cash, by check, Visa, Master Card, American Express or Diners cards.

EMERGENCY

In cases of emergencies when you need immediate medical attention or transportation to the hospital, call 911!

You may call our office during regular business hours at 319-754-4400. If the office is closed, please contact the physician on call by calling Great River Medical Center at 319-768-1000 and have the operator page the physician on call if available.

APPOINTMENTS

Our staff is pleased to schedule appointments for patient referrals from medical professionals only, i.e. MD, DO, DC, DDS or psychologist.

We ask you to notify the office at least 24 hours in advance if the appointment cannot be kept so accommodations can be made for others.
